Appointments

Our notary services are by appointment only to ensure you receive prompt and dedicated assistance. Please call the library at 203-426-4533 or email notary@chboothlibrary.org to make an appointment with a notary.

What to Expect During Your Visit

  • Meet with the notary to review your document and ensure that it can be notarized.
  • Sign the document in front of the notary. The notary must witness your signature; please do not arrive with the document already signed.
  • The notary will sign and stamp your document.
  • The notary will record the details of the notarization in their log book.

Each notarization takes approximately 10 minutes.

Please check if your document requires witnesses. If it does, bring them with you, as the library cannot provide witnesses.

  • Subpoenas
  • Vital records (birth, death, marriage certificates)
  • Land records
  • Medical or health directives

Documents We Can Notarize

  • Job applications
  • Permits and renewals
  • Divorce applications
  • Affidavits
  • Travel consent forms
  • Unclaimed property reports

What to Bring

  • Unsigned document(s) to be notarized
  • Two forms of valid ID:
    • One government-issued ID with a photo
    • A secondary ID that includes a signature

Acceptable Forms of ID

  • Primary: Driver’s license, Passport
  • Secondary: Any of the primary forms, signed credit card, signed library card

Unacceptable Forms of ID

  • Social Security card
  • Birth certificate
  • Marriage license
